Acne treatment that you apply to the skin: Most acne treatments are applied to the skin. Your dermatologist may call this topical treatment. There are many topical acne treatments. Some topicals help kill the bacteria. Others work on reducing the oil. The topical medicine may contain a retinoid, prescription-strength benzoyl peroxide, antibiotic, or even salicylic acid. Your dermatologist will determine what you need.

Topical antibiotics deemed safe during pregnancy include clindamycin, erythromycin, and metronidazole (all category B), due to negligible systemic absorption. Nadifloxacin and dapsone (category C) are other topical antibiotics that may be used to treat acne in pregnant women, but have received less study. No adverse fetal events have been reported from the topical use of dapsone. If retinoids are used there is a high risk of abnormalities occurring in the developing fetus; women of childbearing age are therefore required to use effective birth control if retinoids are used to treat acne. Oral antibiotics deemed safe for pregnancy (all category B) include azithromycin, cephalosporins, and penicillins. Tetracyclines (category D) are contraindicated during pregnancy as they are known to deposit in developing fetal teeth, resulting in yellow discoloration and thinned tooth enamel. Their use during pregnancy has been associated with development of acute fatty liver of pregnancy and is further avoided for this reason.

Microneedling is a procedure in which an instrument with multiple rows of tiny needles is rolled over the skin to elicit a wound healing response and stimulate collagen production to reduce the appearance of atrophic acne scars in people with darker skin color. Notable adverse effects of microneedling include postinflammatory hyperpigmentation and tram track scarring (described as discrete slightly raised scars in a linear distribution similar to a tram track). The latter is thought to be primarily attributable to improper technique by the practitioner, including the use of excessive pressure or inappropriately large needles.
Frankincense oil is a personal favorite for me and my wife, Chelsea. Containing antibacterial and anti-inflammatory properties, it’s amazing for almost all skin types and perfect for acne-prone skin. Frankincense invites new cell growth, which can help reduce the appearance of scars. It also helps prevent or eliminate bacteria, part of what can cause acne in the first place.
There has been a long-observed link between higher stress levels and the incidence of breakouts, and studies have shown that stress can worsen acne’s frequency and severity. Sebaceous glands contain receptors for stress hormones, making them upregulated and kicking sebum production into overdrive. Unfortunately, those with stress sometimes fall victim to a vicious acne cycle. Anxious types have a tendency to pick their skin and pop pimples under stress. This bad habit can exacerbate blemishes by pushing the buildup deeper into the pore, inducing cellular damage, rupturing cellular walls, and spreading bacteria. In extreme cases, sometimes people become so worried or embarrassed about their skin that they compulsively pick at every little thing that shows up. This condition is called acne excoriee, and can turn mild acne into severe scars.
Lavender oil is another favorite of mine, not only because it can help prevent and keeps acne at bay, but it smells amazing, and provides relaxation qualities — something much needed in today’s world. Lavender helps regenerate skin cells, minimize sun spots and even reduce scarring caused by acne. Additionally, it can help reduce swelling and inflammation that may be caused by acne due to the polysaccharides it contains. (20)
Like baby acne, eczema is very common in newborns, but it’s usually caused by dry (not oily) skin. Mild eczema can cause lots of small bumps, similar to baby acne. Since it often starts on the cheeks, it’s easy to confuse with baby acne. But, if it spreads into a rash that covers the skin or develops in other areas of your baby’s body, such as the folds of his elbows and knees, then it’s probably eczema, not acne. Eczema isn’t usually serious. You can treat it by using a gentle hydrating lotion on your baby’s skin after bathtime.
The verdict on smoking and its relationship to acne is still undecided. The evidence goes back and forth; while many studies seem to prove this theory, other studies contradict such research. For example, research published in 2001 by the British Journal of Dermatology concluded that out of 896 participants, smokers tended to have more acne in general; the more they smoked, the worse their acne felt and appeared. Confusingly, a study published just five years later in the Journal of Investigative Dermatology offered opposing research; nurses interviewed over 27,000 men within a span of 20 years and found that active smokers showed significantly lower severe than non-smokers. Although smoking’s relationship with acne vulgaris is undetermined, smoking has proven effects on acne inversa. It also disrupts hormonal balance, lowers vitamin E levels (an essential antioxidant in skin), induces higher instances of psoriasis, decreases oxygen flow to skin cells, and slows the healing process of open sores. Acne aside, smoking promotes wrinkles and premature aging. Did we mention it’s also deadly? Kick this habit to the curb; your skin will thank you.
How to Handle It: Speaking of touching, don't! Picking it, squeezing it, or poking at it will only worsen the situation. These may disappear on their own after a few days. Otherwise, Zeichner suggests visiting your dermatologist for a shot of cortisone, which will reduce inflammation and shrink it in just 24 to 48 hours. But if a last-minute appointment isn't in the cards, play mad scientist. First, ice the area, and then apply salicylic acid gel, benzoyl peroxide gel, and 1 percent hydrocortisone cream. The combo will calm skin, kill bacteria, and draw out excess oil from the pimple — all things necessary to take this down, says Zeichner.
Fractional laser treatment is less invasive than ablative laser treatment, as it targets only a fraction of the skin at a time. Fractional lasers penetrate the top skin layers, where its light energy stimulates collagen production and resurfaces the top layer of the epidermis. Treatments typically last between 15 and 45 minutes and effects become visible in 1 to 3 weeks.
Topical and oral preparations of nicotinamide (the amide form of vitamin B3) have been suggested as alternative medical treatments. It is thought to improve acne due to its anti-inflammatory properties, its ability to suppress sebum production, and by promoting wound healing. Topical and oral preparations of zinc have similarly been proposed as effective treatments for acne; evidence to support their use for this purpose is limited. The purported efficacy of zinc is attributed to its capacity to reduce inflammation and sebum production, and inhibit C. acnes. Antihistamines may improve symptoms among those already taking isotretinoin due to their anti-inflammatory properties and their ability to suppress sebum production.
Comedones associated with acne vulgaris are usually easy to clear using OTC products, but papules and pustules are a little tougher to treat. Since they arise from trapped, infected pores, the first step in their treatment is to remove the inflammation and reduce the swelling so the pore can heal and breathe. Nodules are one of the few acne conditions that actually demand treatment, as some risk the potential of becoming cancerous. Medical treatment with traditional antibiotic therapy will relieve the symptoms of your acne nodule and gradually decrease its size.
C. acnes also provokes skin inflammation by altering the fatty composition of oily sebum. Oxidation of the lipid squalene by C. acnes is of particular importance. Squalene oxidation activates NF-κB (a protein complex) and consequently increases IL-1α levels. Additionally, squalene oxidation leads to increased activity of the 5-lipoxygenase enzyme responsible for conversion of arachidonic acid to leukotriene B4 (LTB4). LTB4 promotes skin inflammation by acting on the peroxisome proliferator-activated receptor alpha (PPARα) protein. PPARα increases activity of activator protein 1 (AP-1) and NF-κB, thereby leading to the recruitment of inflammatory T cells. The inflammatory properties of C. acnes can be further explained by the bacterium's ability to convert sebum triglycerides to pro-inflammatory free fatty acids via secretion of the enzyme lipase. These free fatty acids spur production of cathelicidin, HBD1, and HBD2, thus leading to further inflammation.
Light therapy is a treatment method that involves delivering certain specific wavelengths of light to an area of skin affected by acne. Both regular and laser light have been used. When regular light is used immediately following the application of a sensitizing substance to the skin such as aminolevulinic acid or methyl aminolevulinate, the treatment is referred to as photodynamic therapy (PDT). PDT has the most supporting evidence of all light therapies. Many different types of nonablative lasers (i.e., lasers that do not vaporize the top layer of the skin but rather induce a physiologic response in the skin from the light) have been used to treat acne, including those that use infrared wavelengths of light. Ablative lasers (such as CO2 and fractional types) have also been used to treat active acne and its scars. When ablative lasers are used, the treatment is often referred to as laser resurfacing because, as mentioned previously, the entire upper layers of the skin are vaporized. Ablative lasers are associated with higher rates of adverse effects compared with nonablative lasers, with examples being postinflammatory hyperpigmentation, persistent facial redness, and persistent pain. Physiologically, certain wavelengths of light, used with or without accompanying topical chemicals, are thought to kill bacteria and decrease the size and activity of the glands that produce sebum. The evidence for light therapy as a treatment for acne is weak and inconclusive. Disadvantages of light therapy can include its cost, the need for multiple visits, time required to complete the procedure(s), and pain associated with some of the treatment modalities. Various light therapies appear to provide a short-term benefit, but data for long-term outcomes, and for outcomes in those with severe acne, are sparse; it may have a role for individuals whose acne has been resistant to topical medications. A 2016 meta-analysis was unable to conclude whether light therapies were more beneficial than placebo or no treatment, nor how long potential benefits lasted. Typical side effects include skin peeling, temporary reddening of the skin, swelling, and postinflammatory hyperpigmentation.
“You unfortunately cannot determine the strength of a product strictly by the percentage of its active ingredients because how well a product works depends on how well its inactive ingredients help it penetrate the skin,” explains Dr. Green. “In other words, a 2 percent benzoyl peroxide may be more effective than another brand’s 5 percent benzoyl peroxide because there are other ingredients helping out.”
Acne inversa (L. invertō, "upside down") and acne rosacea (rosa, "rose-colored" + -āceus, "forming") are not true forms of acne and respectively refer to the skin conditions hidradenitis suppurativa (HS) and rosacea. Although HS shares certain common features with acne vulgaris, such as a tendency to clog skin follicles with skin cell debris, the condition otherwise lacks the defining features of acne and is therefore considered a distinct skin disorder.

Hormonal activity, such as occurs during menstrual cycles and puberty, may contribute to the formation of acne. During puberty, an increase in sex hormones called androgens causes the skin follicle glands to grow larger and make more oily sebum. Several hormones have been linked to acne, including the androgens testosterone, dihydrotestosterone (DHT), and dehydroepiandrosterone (DHEA); high levels of growth hormone (GH) and insulin-like growth factor 1 (IGF-1) have also been associated with worsened acne. Both androgens and IGF-1 seem to be essential for acne to occur, as acne does not develop in individuals with complete androgen insensitivity syndrome (CAIS) or Laron syndrome (insensitivity to GH, resulting in very low IGF-1 levels).
In women, acne can be improved with the use of any combined birth control pill. These medications contain an estrogen and a progestin. They work by decreasing the production of androgen hormones by the ovaries and by decreasing the free and hence biologically active fractions of androgens, resulting in lowered skin production of sebum and consequently reduced acne severity. First-generation progestins such as norethindrone and norgestrel have androgenic properties and can worsen acne. Although oral estrogens can decrease IGF-1 levels in some situations and this might be expected to additionally contribute to improvement in acne symptoms, combined birth control pills appear to have no effect on IGF-1 levels in fertile women. However, cyproterone acetate-containing birth control pills have been reported to decrease total and free IGF-1 levels. Combinations containing third- or fourth-generation progestins including desogestrel, dienogest, drospirenone, or norgestimate, as well as birth control pills containing cyproterone acetate or chlormadinone acetate, are preferred for women with acne due to their stronger antiandrogenic effects. Studies have shown a 40 to 70% reduction in acne lesions with combined birth control pills. A 2014 review found that antibiotics by mouth appear to be somewhat more effective than birth control pills at decreasing the number of inflammatory acne lesions at three months. However, the two therapies are approximately equal in efficacy at six months for decreasing the number of inflammatory, non-inflammatory, and total acne lesions. The authors of the analysis suggested that birth control pills may be a preferred first-line acne treatment, over oral antibiotics, in certain women due to similar efficacy at six months and a lack of associated antibiotic resistance.
Scars (permanent): People who get acne cysts and nodules often see scars when the acne clears. You can prevent these scars. Be sure to see a dermatologist for treatment if you get acne early — between 8 and 12 years old. If someone in your family had acne cysts and nodules, you also should see a dermatologist if you get acne. Treating acne before cysts and nodules appear can prevent scars.
