The verdict on smoking and its relationship to acne is still undecided. The evidence goes back and forth; while many studies seem to prove this theory, other studies contradict such research. For example, research published in 2001 by the British Journal of Dermatology concluded that out of 896 participants, smokers tended to have more acne in general; the more they smoked, the worse their acne felt and appeared. Confusingly, a study published just five years later in the Journal of Investigative Dermatology offered opposing research; nurses interviewed over 27,000 men within a span of 20 years and found that active smokers showed significantly lower severe than non-smokers. Although smoking’s relationship with acne vulgaris is undetermined, smoking has proven effects on acne inversa. It also disrupts hormonal balance, lowers vitamin E levels (an essential antioxidant in skin), induces higher instances of psoriasis, decreases oxygen flow to skin cells, and slows the healing process of open sores. Acne aside, smoking promotes wrinkles and premature aging. Did we mention it’s also deadly? Kick this habit to the curb; your skin will thank you.
Apricot seeds may be a great option for how to get rid of pimples. A recent study published in Phytotherapy Research notes that the phytonutrients and antimicrobial qualities of apricot essential oil obtained from apricot seeds may help provide glowing skin. Some people use natural treatments like tea tree oil (works like benzoyl peroxide, but slower) or alpha hydroxy acids (remove dead skin and unclog pores) for their acne care. Many different treatments exist for acne. These include alpha hydroxy acid, anti-androgen medications, antibiotics, antiseborrheic medications, azelaic acid, benzoyl peroxide, hormonal treatments, keratolytic soaps, nicotinamide, retinoids, and salicylic acid. Salicylic acid is a topically applied beta-hydroxy acid that stops bacteria from reproducing and has keratolytic properties. It opens obstructed skin pores and promotes shedding of epithelial skin cells. Salicylic acid is known to be less effective than retinoid therapy. Dry skin is the most commonly seen side effect with topical application, though darkening of the skin has been observed in individuals with darker skin types.
A nodule is an abnormal tissue growth which can either develop just below the skin or anywhere within the skin’s three layers (the epidermis, dermis, and subcutaneous tissue). Nodules commonly form in regions such as the face, neck, armpits, and groin, although they can also develop on internal organs such as the lungs, thyroid, and lymph nodes. They create solid, raised lumps that are more than 1 to 2 centimeters in diameter, with the potential to reach up to the size of a hazelnut. Nodules are hard and firm to the touch, unlike cysts whose pus makes them softer to the touch. This type of severe acne should be consulted by a doctor, as it might be indicative of a more serious condition.
Blackheads appear as small black dots on the skin. They usually occur on the face – especially the nose and T-Zone – but can also be seen on the back, neck, chest, shoulders and arms. Blackheads are one of two types of comedones, or acne lesions caused by clogged pores. Blackhead comedones are open versus closed, leaving the plug at the top of the clogged pore exposed to air on the skin’s surface. It’s this exposure to oxygen that accounts for their color, which can not only be black but also gray, yellow or brown. When melanin – a pigment produced by oil glands and found within sebum – makes contact with air, it oxidizes and turns dark. Blackheads are a mild, usually painless form of acne, as there’s less inflammation associated with this type of lesion.
Scientists initially hypothesized that acne represented a disease of the skin's hair follicle, and occurred due to blockage of the pore by sebum. During the 1880s, bacteria were observed by microscopy in skin samples affected by acne and were regarded as the causal agents of comedones, sebum production, and ultimately acne. During the mid-twentieth century, dermatologists realized that no single hypothesized factor (sebum, bacteria, or excess keratin) could completely explain the disease. This led to the current understanding that acne could be explained by a sequence of related events, beginning with blockage of the skin follicle by excessive dead skin cells, followed by bacterial invasion of the hair follicle pore, changes in sebum production, and inflammation.
Benzoyl peroxide attacks the P. acnes bacteria. However, one of its main side effects is dryness: If you’re going to use anything with benzoyl peroxide, make sure to moisturize afterwards. Sulfur and azelaic acid are less common and less severe alternatives to benzoyl peroxide. Dr. Peter Lio, assistant professor of clinical dermatology at Northwestern University, says sulfur-based treatments are “a good fit for patients who can’t tolerate the side effects of benzoyl peroxide.”
Washing your face with regular soap isn't enough to make acne better. The best face wash for acne is effective at removing oil and dirt, but still gentle enough to use regularly without overdrying your skin. Look for topical acne medication ingredients salicylic acid and/or benzoyl peroxide in your face wash and use gentle, nonabrasive cleansing techniques.
The earliest pathologic change is the formation of a plug (a microcomedone), which is driven primarily by excessive growth, reproduction, and accumulation of skin cells in the hair follicle. In normal skin, the skin cells that have died come up to the surface and exit the pore of the hair follicle. However, increased production of oily sebum in those with acne causes the dead skin cells to stick together. The accumulation of dead skin cell debris and oily sebum blocks the pore of the hair follicle, thus forming the microcomedone. This is further exacerbated by the biofilm created by C. acnes within the hair follicle. If the microcomedone is superficial within the hair follicle, the skin pigment melanin is exposed to air, resulting in its oxidation and dark appearance (known as a blackhead or open comedo). In contrast, if the microcomedone occurs deep within the hair follicle, this causes the formation of a whitehead (known as a closed comedo).
The side effects depend on the type of treatment you use. Generally, for topical, over-the-counter creams, you can watch out for stinging, redness, irritation and peeling — these side effects usually don’t go any deeper than the skin. Others, like oral antibiotics or hormonal medications, could come with new sets of complications, so we suggest talking to your doctor before pursuing the treatment.
Every expert we spoke with said the most critical part of combating acne is combating it every day. “The only way to make any medication work is to use it on a daily basis,” says Dr. Green. Aesthetician and Rodan + Fields Consultant, Jessica Fitz Patrick emphasizes that it really comes down to what you can maintain for the long term: “Kits are great because they take out all the guesswork -- you just follow the instructions. But if four steps is going to be too many for you to keep up week after week, you’ll be better off finding one that has fewer treatments.”
